Nature of BenefitA maximum financial assistance of ₹50,000/- to each beneficiary for surgery, excluding government funds, will be deposited in the bank account of the respective beneficiary.

Required DocumentsCompleted application form, certificate from the health department (regarding serious illness and surgery), Aadhar card, local resident proof (e.g. ration card), Poverty Line Number and certificate or birth certificate of the Tehsildar, copy of Gram Sabha resolution if it is a tribal area, copy of bank passbook.

Method of ApplicationThe fully filled application form should be submitted to the Child Development Project Officer through the Anganwadi Worker. At the district level, all applications should be collected from the Child Development Project Officer and submitted for approval by the Women and Child Welfare Committee.
